Pre-Registration Form
Please print out the form and complete it. Then send both the completed form and the appropriate deposit (see prices) to the Party Pony address located below. Your child's spot will be reserved when we receive the form/deposit. Please have your deposits and pre-registration form turned in by May 10th, as your other paperwork is due by May,30th.

After your child's spot at camp is confirmed, please print out and complete this form. As soon as completed, please send us the required camp forms at the address located below. Your paperwork must be processed by May 30th, so please send your paperwork in as soon as possible. Please send in with the "Liability Release Form".
